Public Disclosure Forms
All council members are required to complete three public disclosure forms throughout their term.
These forms:

- Original Statement (one to be completed and attached to nomination papers prior to election, and one to be completed within 30 days of being elected)
- Annual Declaration (by November 30 every year, a Municipal Revenue Sharing requirement)
- Statement Amendment (any time a council member feels that his/her Public Disclosure Statement needs to be updated due to a new circumstance)

- Assist with transparency and accountability as pillars of good governance
- Assist the public’s understanding of any dealings that may compromise the council member’s ability to make decisions
